1) Make sure your device is able to scan QR codes- check the settings on your device, go into your Camera app and make sure the “Scan QR codes” is activated. 

2) Open your Camera app (make sure you are on “Photo” mode instead of “Video” mode) and make sure your camera is not on “Selfie” mode (you can see the QR code from your screen, not your face).

3) Hold the phone over the QR code (you will see 4 corners on your phone you can use to fit over the code). Your phone will recognize the QR code and a notification (thereceptionist.com) will appear at the top (or bottom) of your phone screen. 

4) Tap the notification that comes up and open up the webpage associated with the QR code. 

5) Search for your clinician in the Three Oaks Wellness menu or type in your clinician’s name in the search bar

6) Once you click on your clinician, you will be prompted to write/ type in your first name. 

7) After typing in your first name, you will be directed to a COVID-19 Screening page. If you are symptom free, click the green “I Agree” button and your clinician will be notified that you have checked in for your appointment.

8) Wait in the waiting room for your clinician to come and escort you back into their office.

9) If you have QR or other scanning apps, just follow steps 3-8.
